* Due to the COVID-19 pandemic, our HEAP Utility Assistance services are ONLY available via MAIL-IN and ONLINE submissions (please refer to MAIL-IN instructions below the HEAP section, refer to above for ONLINE).

HEAP Application Mail-In Instructions
1) Please complete & attach ALL of the REQUIRED forms listed in the HEAP Document Checklist (Below) along with the completed Utility Assistance applications (PDF's listed at the bottom of this section)
2) After completion, please mail the application and ALL required documents to the following address:

3) We will contact you with a follow-up for your application as soon as we receive it in our corporate office. Additionally, we have a drop-box at our location located outside of the front office door.
HEAP DOCUMENT CHECKLIST
* Application MUST include ALL documents listed below
1. Current State ID / Driver's License (Applicant ONLY)
2. Social Security Number (Applicant ONLY)
3. Current Bill: Electric/Gas/Propane (Most recent. Must send ALL pages of utility bills. BOTH bills are required)
4. Rent / Mortgage (Most current receipt)
5. Signed Consent Form (Utility Account Holder) On both bills. NOT required if the applicant's name is on the bill
6. Cal Fresh / Food Stamp Verification (Current / Last Month)
7. Current Household Income:
Everyone living in the household 18+ years old MUST present proof of income.
If household members DO NOT have income, CSD 43B form must be filled out.

- Loans (Last 4 Weeks. State & Sign Form 43B)
Self-employed, Cash income, Cash/Loan Assistance MUST be addressed in the attached affadavit.

LIHEAP Application Submission Instructions
1) Please complete & attach ALL of the REQUIRED forms listed in the LIHEAP Document Checklist (Below) along with the completed Weatherization applications (PDF's listed at the bottom of this section).
2) After completion, please submit the application and ALL required documents. You can submit through the following method:

LIHEAP DOCUMENT CHECKLIST
1. Current Electric Bill (last 6 weeks; send copies of all pages)
2. Current Gas Bill (last 6 weeks; send copies of all pages)
3. Current Household Income
* Everyone living in the household 18+ years old must present proof of income.
If a household member DOES NOT have an income, CSD 43B form must be filled out.
- For Self-Employed, Cash Income, Cash/Loan Assistance: send an affadavit / letter stating this situation
- For Self-Employed: send copy of current tax forms
- For Section 8 or HUD: send copy of the voucher program / contract verification
- For Property Managers that are authorized to sign forms: send copy of Property Owner Authorization Letter
